When to consider Home Health or Hospice Care

• When you are calling your primary care provider frequently with questions
• When you see a sudden decline in your loved one’s condition
• When there is an increase in symptoms
• When your loved one has experienced multiple hospitalizations
• When you or your loved one need assistance with medication management or activities of daily living
